“Act on the Protection of Personal Information”

Pursuant to the “Act on the Protection of Personal Information,” we disclose the following items regarding the handling of personal information.

1.Purpose of Use

We use personal information for the following purposes in our business activities, including the manufacture, sale, and import/export of veterinary medicines, feed and feed additives.

2.Safety Management Measures

We take the following measures to ensure proper management of personal information and other personal data, including preventing its leakage, loss or damage.

Establishment of basic policy
We have established guidelines for protecting personal information in order to ensure this information is handled appropriately and to inform you of the contact point for inquiries, e.g., regarding our handling of personal information.

Establishment of rules regarding the handling of personal data
We have established personal information protection regulations which cover the handling methods, and the persons responsible and their duties, for each stage of handling, including acquisition, use, storage, provision and deletion/disposal.

Organizational safety management measures
Each member of the ASKA Pharmaceutical Group…
• Has appointed a person who is responsible for handling personal data.
• Has clearly designated which employees handle personal data and what the scope of the personal data they handle will be.
• Has established a reporting and communication system in the event that an incident, such as a leak of personal data, occurs or there are signs of such an incident.
• Ensures its management department appropriately supervises the handling of personal data.

Personnel-based safety management measures
• We provide training to our employees on important points regarding the handling of personal information.
• We include stipulations about personal information confidentiality-related matters in work regulations.

Physical safety management measures
• We store documents containing personal information and other important information in lockable cabinets.
• We require users to verify their identity by entering a password when starting up a PC or smartphone.

Technology-based safety management measures
• We implement appropriate access controls to manage who is in charge of personal information and the scope of the personal information he or she handles.
We have taken measures to ensure that software used for business purposes is updated appropriately. Also, we have installed firewalls and antivirus software to enable constant, system-wide monitoring.

Ascertainment of the external environment
• We do not handle any personal information in foreign countries.

3.Joint Use

In line with the following, we may share personal information for joint use within the ASKA Pharmaceutical Holdings Group.

History and content of inquiries, comments, opinions, etc., regarding our products or services (including name, address, telephone number, email address, and other contact information, product or service names, etc.)

ASKA Pharmaceutical Holdings Group companies (ASKA Pharmaceutical Holdings Co., Ltd., ASKA Pharmaceutical Co., Ltd., ASKA Animal Health Co., Ltd., ASKA Pharma Medical Co., Ltd.)

Responding to inquiries, comments, opinions, etc.
Considering improvements in our products or services

4.Provision to Third Parties

With the consent of the individual in question, we may provide in writing, on electromagnetic recording media, or by email or other means, an individual's personal information which we possess, such as his or her name, address, telephone number, or name of affiliated company, organization or department, to our subsidiaries, business partners or other third parties in order to achieve the Purposes of Use described in 1. The individual may request that we stop the provision of this information to third parties.

6. Procedures for Disclosure, Correction, Suspension of Use, Etc.

We are responsive to requests for disclosure, correction, suspension of use, etc., of retained personal data (“Disclosure Requests”) which are made by the person to whom the personal data pertains or by his or her representative.

1. Items subject to Disclosure Requests
Items of retained personal data that are subject to disclosure include name, address, gender, email address, telephone number, etc.
2. Documentation required for Disclosure Requests
When making a Disclosure Request, please download the following request form (A), fill in all the required information, enclose identity verification documentation (B), and mail it to the Personal Information Inquiry Desk.

A copy of your driver's license, passport, health insurance card, or pension book or other such official document
Note: If you are unable to print out the request form (A) from our website, please contact our Personal Information Inquiry Desk by mail and request that we send it to you.
3. Disclosure Request from a representative
For Disclosure Requests made by representative, the documents listed below should be included in addition to the documents listed in (2) above.
・Statement of power of attorney
・Certificate of personal seal used to create the power of attorney
・Identity verification documentation for the representative
 A copy of the representative’s driver's license, passport, health insurance card, or pension book or other such official document
4. Fee and collection method
There is a fee of 800 yen per request.
Please enclose 800-yen worth of postage stamps with your request.
*If the fee is insufficient or not enclosed, we will contact you; however, if payment is not made within the specified period, we will treat the Disclosure Request as if it had not been made.
5. Where to send Disclosure Requests
For Disclosure Requests, please enclose the necessary documentation together with the specified request form and mail it to the Personal Information Inquiry Desk. Please also write “Disclosure Request Documents Enclosed” in red ink on the envelope.
6. Method of response for Disclosure Requests
We will respond in writing to the requesting party’s address listed on the request form.
7. Purpose of Use of personal information obtained through Disclosure Request
Personal information obtained in connection with a Disclosure Request will be handled only to the extent necessary to respond to the Disclosure Request. After the response is completed, the submitted documents will be stored for two years and then discarded.

*Reasons for non-disclosure of retained personal data
We will not disclose personal data in the following cases. In the event that we decide not to disclose personal data, we will notify you of this decision and the reasons. The specified fee will be charged regardless of whether the data is disclosed.

1. If the identity of the individual cannot be confirmed, such as when the address on the request form, the address on the identity verification documentation, or the address registered with us do not match
2. If any of the specified documentation is incomplete
3. If the object of the Disclosure Request does not fall under the category of retained personal data
4. If there is a risk of harm to the life, body, property or other rights and interests of the individual in question or a third party
5. If there is a risk of causing significant disruption to the proper performance of our business
6. If disclosure would violate other laws and regulations
